– Tooth 13 slightly intruded (about 3 mm) so it can be repositioned using forceps
– Tooth 11 with crown fracture involving the pulp
– Tooth 22 with crown fracture at the distal incisal edge
– Teeth 11, 21, and 22 luxated due to a horizontal alveolar fracture
– Tooth 15 with a palatal cusp fracture that is paracrestal
– Vestibular laceration (as shown in the reference photo) to allow for suturing
– Height of the alveolar fractures: 3–4 mm from the apex of each tooth, so they can be luxated as a block
– Vertical fracture extending 4 mm apical to the gingival margin
